This form is for existing clients of GVH/GCH with animals on long-term medication. If you would like to request more medication for your animal, please fill in the form below using the information on the label of your previously dispensed medication and submit it using the SUBMIT button at the bottom. If you do not have a label from previously dispensed medication to hand, please call us to process your medication request on (02) 6242 7276 instead of proceeding with the form below.
If you need to request more than one type of medication, please complete and submit a separate repeat medication request form for each medication.
We will only supply ongoing medications which have already been dispensed by GVH/GCH, for this particular animal, and for this particular condition. We prepare medications between 11.00am and 1.00pm on weekdays, and 11.00m to 12.00pm on Saturdays. Requests received after these starting times will be held over to the following day.
You will receive an acknowledgement via email when we receive your request. Please only call or send an email if you have not received a return email within 6 hours of submission through normal work hours or 3 hours of our next opening time for submissions made out of hours. You will receive a notification via SMS when your medication is ready to collect.
We reserve the right to refuse a request to dispense medications. We must have an ongoing relationship with you and your animal and believe that we can dispense the medications without the need for a medical review. A review may be requested, and in this case, the request for dispensing may be refused until the review is undertaken.
